Blake Hampton

Licensed Massage Practitioner


Blake Hampton attended the Brian Utting School of Massage in Seattle, WA and graduated from the school’s 1000-hour program. He has been working as a massage therapist in private practice since 1995 and as personal trainer and group exercise instructor for many area gyms. He attended Cincinnati Conservatory of Music where he graduated with a BFA in Musical Theater and Dance. He is a professionally trained dancer and choreographer who has performed with many of the country’s most prestigious dance companies as well as Broadway touring shows. He has also been a competitive swimmer, diver and gymnast. His passions include hiking, yoga, cooking, organic gardening and his two beautiful pets, Shasta and Sugar.

Mike Clark

Licensed Massage Practitioner


Mike Clark was born in San Francisco, California and grew up in Southeast Asia. Having been raised in Asia, Michael has developed a profound interest and respect for Eastern forms of bodywork and medicine which gives him a unique ability to connect the body and mind through a peaceful, healing and often spiritual journey. Upon returning to the U.S., he attended the Mueller College of Holistic Studies in San Diego, CA and graduated with top honors. He has been in private practice since 1995. He loves to travel to different countries and is intensely curious about other cultures. He has been surfing since age 4 and has competed in swimming and equestrian events. He is passionate about yoga, snowboarding, hiking, music, fine wines and good food.

The Fine Print

Missed Appointments and Cancellation Policy:

Our time together is important. If you must cancel your appointment and it is not an emergency and you are not sick, please give at least 24 hours notice. Late cancellations and no-shows will be charged full price. We have the right to determine what constitutes an Emergency Cancellation and when to appropriately charge for your late or missed appointment.


Late Arrivals Policy:

If you arrive late for an appointment, the remaining time will be used to its best advantage. However, you will still be charged the same session price as if you arrived on time. Punctuality will assure full use of your allotted appointment time. If you are going to be more than 5 minutes late, please call as soon as you can. If you are more than 15 minutes late without a call, you will be considered a “No Show”. Missed appointments or clients who run late are a hardship on your therapist and unfair to other clients who have appointments scheduled after you. If your therapist is running late, you will still receive your full session time.


Check Policy:

We accept cash at the beginning of your session and you may tip at the end if you feel so inclined. We do not accept personal or company checks, traveler’s checks or cashier checks. You may also pre-pay on our website via PayPal.  All major credit cards are accepted (Visa, MC, American Express or Discover Cards). We do not take credit cards at the time of your session.


If You’re Under 18 or Appear To Be Under 18:

For your protection and for the protection of our therapists, we do not see anyone under the age of 18. If you appear to be under the age of 18, we have the right to ask for age verification and you must present a driver’s license or birth certificate before we will work on you. We do not make exceptions to these rules.


Right to Decline a Massage Due to Contraindications:

Under certain medical conditions, massage and bodywork are not advised (for example: recent stroke, heart-attack, surgery or chronic illness). Massage is potentially harmful to a client with a serious acute or chronic medical condition. We have the right to decline service if we feel your safety or health is at risk. If you are unsure if your condition indicates or contraindicates massage, please call your doctor or contact us prior to your visit. If we decline to massage you because of a contraindication, you will not be charged.


Right to Decline Services:

At any time, we have the right to decline service or to cancel your massage during the session for any reason.


Sick/Illness Policy:

Please do not schedule a massage if you have an acute or a contagious illness. If on the morning of your scheduled appointment, you are feeling ill or if you are coming down with something, please call to reschedule your session. When your body is compromised, massage can often exacerbate symptoms. It also puts us and our clients at risk of infection and transfer.


Massage Packages - Terms and Conditions:

Massage packages are to be paid in full prior to the first scheduled session. Massage Packages have an expiration of 6 months from the date sold. Massage Packages are non-transferable and non-refundable.


Your requirements at each visit:

  1. 1.     Our office is an oasis for relaxation and rejuvenation. To maintain this environment, we ask that all of our clients be respectful of other people’s privacy. Kindly speak softly, and deactivate any communication devices, such as cell phones or pagers before you enter the building.

  2. 2.     Our practice is located in a residential apartment building. Please do not knock on other people’s doors if you’re lost. Please do not enter the building without being buzzed in by Mike or Blake.

  3. 3.     Your session begins and ends at your scheduled appointment time.

  4. 4.     If you arrive late, you will be charged the full price of your session.

  5. 5.     Please do not arrive under the influence of alcohol or drugs.

  6. 6.     Your session can be cancelled at any time if your therapist feels that you are under the influence of alcohol or drugs or for any other behavior deemed inappropriate.

  7. 7.     You must provide a credit card to hold your appointment.

  8. 8.     You are responsible for providing, to the best of your knowledge, an accurate and complete health history form.

  9. 9.     You should update your therapist or discuss any changes in your health at the start of each session.

  10. 10.   You are expected to accurately disclose all information necessary for each massage therapy session.

  11. 11.   You are responsible for your therapist’s safety and well-being and should not put your therapist at harm or risk.

  12. 12.   If you need to cancel your session, you must give a 12-hour notice.

  13. 13.   You will be charged the full price of your session if you are late or if you are a “No Show.”

  14. 14.   Emergency cancellations will not be charged for, but are determined at the therapists’ discretion.

  15. 15.   Payment is expected at the time service is rendered; you cannot be billed for sessions.

  16. 16.   You are expected to pay for your session upon arrival; tips or gratuities are accepted after the session ends.

  17. 17.   Harassment of any kind is not tolerated.

  18. 18.   Our massage practices are strictly therapeutic and our relationships with clients are strictly professional. You may experience sensual touch; however, sexual or inappropriate behavior will not be tolerated and the session will be immediately terminated and charged in full.

  19. 19.   If the therapist feels his or her safety is being compromised, the session will be stopped immediately.

  20. 20.   This office is a non-smoking environment.

  21. 21.   Be clean; please shower the same day as your appointment.

  22. 22.   Please avoid wearing strongly-scented body products.

  23. 23.   Because of the deep stretches and varying body positions involved, massage is best performed on an empty stomach. Please allow at least an hour, and preferably 2 to 3 hours between your last meal and your massage appointment.

  24. 24.   Clients are expected to treat their therapist courteously and respectfully at all times.

  25. 25.   All information is kept private and deemed confidential. No information can be shared with others without your written consent.

  26. 26.   If possible, try not to schedule anything right after your session. Allow yourself some downtime to reap the full benefits of your massage.

 
What our clients can expect from their therapist:

  1. 1.     We provide our clients with a competent and professional session each time they come for an appointment, addressing the client’s specific needs for that session.

  2. 2.     We provide our clients with a confidential setting where their privacy is always respected and maintained.

  3. 3.     We are available to our clients between the hours of 8:00 AM to 6:00 PM Monday-Friday.

  4. 4.     Weekend or emergency sessions are by appointment only.

  5. 5.     Clients can reach us by phone or voicemail Monday-Friday. We do not return calls on weekends or holidays.

  6. 6.     We try, to the best of our ability, to make return calls within a 24 to 48 hour timeframe (or less).

  7. 7.     We are closed all major holidays and every weekend.    

  8. 8.     All clients are treated with respect and dignity.

  9. 9.     We charge fair market prices for all services.

  10. 10.   We are not insurance providers and do not perform Medical Massage; we do not provide direct billing for insurance.

  11. 11.   Payment is due at the time of the service.

  12. 12.   We accept cash, VISA, MC, American Express, Discover and Debit Cards

  13. 13.   We perform services for which we are qualified to do. We will try to refer you to appropriate specialists when work is not within our scope of practice.

  14. 14.   We will keep accurate and confidential records and review them before each session.

  15. 15.   We will customize each session to meet your individual needs.

  16. 16.   We will stay current with information and techniques by reading, receiving regular bodywork sessions and attending continuing education seminars each year.

  17. 17.   We will respect all clients regardless of their age, gender, race, national origin, sexual orientation, religion, socio-economic status, body type, political affiliation, state of health or personal habits.

  18. 18.   If we must cancel your session or reschedule your session for any reason, we will do so within 24-hours whenever possible.

  19. 19.   If an emergency arises and we cannot cancel within 24-hours, we will give you a 20% discount on your next session.

  20. 20.   Our equipment, linens and massage supplies are clean and safe.

  21. 21.   We will respect your personal boundaries at all times.
